This image shows the Table Lookups page of the wizard. 
The following descriptive text appears at the top of the wizard dialog:
"Table Lookups allow you to match an uploaded value against another table and use the associated  key value, instead of the uploaded value."

The following fields are available in this page of the wizard:
1. Add the new table lookup for Column with the value RELATIVE_ID (Number) selected. 
2. Lookup Table Ownder with the value HR selected. 
3. Lookup Table Name with the value EMP (table) selected. 
4. Return Column with the value EMPNO (Number) selected.  
5. Upload Column with the value ENAME (Varchar2) selected.  
6. Upload Column 2 with no selection

The pointer is on the Add Lookup button.
